- been part of a life threatening event that
- caused intense fear or, helplessness or horror. Yes/No
2 Have you re-experienced the threatening
event in at least one of the following ways.
3. Repeated, Distressing memories and or
dreams. Yes/No
4. Acting or feeling as if the event was
happening again? (Flashbacks or feeling you are reliving it all over again.?) Yes/No
5. Feelings of intense psychical and /or
emotional distress when you are exposed to
things that remind you of the event. Yes/No
6. Have you or a family member changed their sleeping or eating habits suddenly. Yes/No
7. Have you or a family member suddenly
changed their response to any stressful family events, loses their temper easily over minor
events. Yes/No
8. Any family member suddenly changed their
use of alcohol or started using drugs to help
them cope better? Yes/No
9 Have any family member including the
children, changed their ability to learn and cope
with school work or their normal work? Yes/No
